If you want to have the free blood test, please fill out the form.
Only first 100 registrants get the FREE BLOOD TEST.

  • Screening for liver and kidney diseases

  • Screening for Heart disease

  • Eye Evaluation

  • We will be handing out FREE vitamins to the first 50 people
